Our licensed and committed care givers can help adults who have a medical condition or illness, who have experienced a recent injury or hospitalization, who need skilled health care or rehabilitation, or who require social work services.
Last year, we made almost 20,000 home health visits. We offer home health services 24 hours a day, 7 days a week. We serve towns throughout the Greater Hartford area.
Accessing Hebrew Community Services' Home Health Care agency is easy. Clients or family members just make one call to set up a meeting with one of our case managers, ideally in the client's home. The case manager will then work with each client's physician to develop an individualized health plan to promote the client's well-being and makes it possible for them to remain in their own home. Hebrew Community Services' Home Health Agency is certified by Medicare and Medicaid. Our Home Health Agency is accredited with commendation by the Joint Commission on Accreditation of Healthcare Organizations.
