Skilled Nursing

Hebrew Community Services nurses are trained to meet the special needs of older adults and adults with chronic illness. They provide hands-on care, offer instruction in self-care, preventative health and wellness, and instruct family members on how to help their older relatives achieve and maintain an optimal level of well-being. Rehabilitation

Our staff of rehabilitation therapists, physical and occupational therapists, speech therapists, and consulting physiatrist are available to help adults of any age who have experienced a loss of function due to illness or injury. We customize individual programs to meet individual needs. Our program recommendations might include therapeutic exercise; re-training for activities of daily living such as bathing, dressing, feeding, or walking; speech therapy exercises and communication; or changes in the patient's home or special equipment to enhance mobility and safety. Social Work

Medical social workers will help patients and their families adapt to changes or stresses that might emerge during a period of illness or injury. They provide information regarding the range of financial and community services that are available to people undergoing rehabilitation. Home Health Aides

Home Health Aides provide individualized, personal care to adults in their own homes under the supervision of a home health nurse. Examples of such care include bathing, light housekeeping, feeding, and dressing. In addition, our Home Health Aides are trained to help clients comply with home exercise programs, as established by a physical therapist or registered nurse. Another important function performed by Hebrew Community Services' Home Health Aides is to help maximize the safety and security of clients in their homes, including helping clients implement safety recommendations made by our nurses. Home Care

Our Home Care companions provide non-health related, in-home services. We offer homemaker services such as laundry, meal preparation, and light housekeeping; companion services such as transportation to off-site appointments and assistance with grocery and personal shopping; and chore services such as heavy cleaning.
