Assisted Living Services Agency (“ALSA”)

Hebrew Community Services’ Assisted Living Services Agency is the preferred provider of health care, personal assistance, companion services and private duty services for the members of Hoffman SummerWood Community. The ALSA is licensed by the State of CT Department of Public Health.
 
The ALSA employs an expert team of professional nurses and certified assisted living aides. The nurses are on-site each day Monday - Friday and available on-call 24 hours a day, 7 days a week. The assisted living aides are on site available to assist with care 24 hours every day.

Participate in the ALSA’s complimentary package of care and learn about medications, nutrition, and wellness.  In the event of a medical emergency, ALSA nursing staff is well-informed and can provide immediate care, with your permission. A sample of services:

• A wellness visit provided by a registered nurse 3 times a year
• On-site nursing staff
• Nurse Consultation with your private care physician (PCP)
• Care Conference with a client and family 
• A registered nurse on-call 24 hours a day, seven days a week
• Response to health care related emergencies
• Supervision and on-going training of the ALSA aide staff
